The transition from pediatric to adult medicine (“transition”) is a particularly sensitive phase for patients with rare chronic diseases. A successful transition is crucial to ensure continuity of medical care, prevent therapy interruptions, and secure both the long-term prognosis and quality of life for those affected. This training provides an overview of possible pitfalls, proven concepts, and practical approaches that help physicians accompany the transition process successfully.
